Concerns raised over hospital documents

Medical staff decided a patient should not be resuscitated without informing the family and left another “do not resuscitate” form in a patient’s records with no patient details attached, according to a report.

An inspection of older people’s care at Paisley’s Royal Alexandra Hospital noted concerns about “the interpretation of the use and purpose of ‘do not attempt cardiopulmonary resuscitation’ (DNACPR) documentation”.

They found “no documented evidence” that efforts had been made to consult with the patients or their families about the decision undertaken in two cases.

Inspectors said they were concerned about responsibility for decision-making and communication between some RAH staff.

It was one of 12 areas of improvement highlighted in the report.

Inspectors found little evidence of mental health assessments and no individualised care planned for patients with confusion or challenging behaviour.
